  • 24.5 hours per week covering a 7 day week on a 4 day rotational basis

Suffolk Family Carers is an innovative and creative charity, recognised nationally as the overall GSK Impact Award winner 2019.  We work in partnership with over 100 organisations to deliver a range of services that impact positively on the lives of adult, young adult and young carers.

It is an exciting time for social prescribing in Suffolk with the development of this new ‘Connect for Health’ pilot project.  Suffolk Family Carers is building on our successful Connect for Health programme in Babergh and Mid Suffolk and looking develop and deliver this service within Ipswich Hospital.  This role (known internally as Senior Community Connector) will link very closely to our Connect for Health team within Suffolk Family Carers and across all partnership areas.

The Connect for Health programme supports people to improve their health and wellbeing, connect with their local community and find their own solutions.

  • Are you passionate about helping people to identify what matters to them? 
  • Are you looking for a new and exciting challenge, to utilise your health coaching/coaching skills?
  • Would you enjoy developing a new project to help reduce isolation and loneliness?

As a Senior Community Connector, you will work one-to-one with people within the hospital and the community to aid a smooth discharge. We are looking for a Senior Community Connector with the experience, skills and confidence to engage people and build positive links within the hospital and local communities.

Suffolk Family Carers is located on a rural business park, easily accessible from the A14 and 4 miles from Ipswich.
